The interrelationships between somatic and psychiatric complaints involve virtually every major organ system and every psychiatric diagnostic category. Psychiatric distress frequently finds expression in physical ailment or pain, and chronic physical illness or disability is a common exacerbant of psychiatric symptoms. The complexity of the interplay between psychological and physiological disease is profound, and many of the mechanisms involved remain undefined or imperfectly understood. Our intention in assembling this volume, the fifth in the Baylor Psychiatry Series, has been to consolidate the most recent medical and psychiatric opinion on the description and treatment of psychophysiological disorders. We have included reviews and new material on disorders long understood to be in the psychophysiological realm, such as gastrointestinal disease, pulmonary reactions, headache, and hypertension. Additionally, there are contributions on conditions less frequently, but no less clearly, recognized as psychophysiological or psychosomatic, including immune response deficits, epilepsy, stuttering, and sleep disorders. Chapters on Couvade syndrome, menstrually related mood disturbance, obesity, keratoconus, and anorexia nervosa examine the degree to which psychological and physical events interact in conditions usually considered attributable essentially to a single system. Drs. Dowling and Gaitz have written excellent reviews of developmental elements in psychophysiological events, and examine such disorders as they are specific to the very young or the very old. Treatment approaches to the variety of disorders are discussed in chapters by Dr. Decker, who describes multifaceted modalities, Dr. Lomax, who examines psychotherapeutic techniques, and in a final chapter on psychopharmacological mitigation of psychophysiological syndromes. Since 1952, postgraduate courses for practising physicians and speci alists have been given by the Medical Faculty of the University of Leiden in the Boerhaave Quarter, in which most of its clinics and laboratories are located. During these years, recent advances in a wide variety of m~dical fields and subjects have been discussed by distin guished speakers from many countries. The steadily increasing atten dance has shown that, as could be expected from the rapid progress of modern medicine, there is a widely felt need for this form of postgra duate study. In 1957, therefore, the Leiden Medical Faculty appointed a permanent committee for the organization of postgraduate medical education. Of the courses given since then, certain material proved to have sufficient immediate scientific value to justify publication, and it now gives the Committee great pleasure to announce that in collaboration with the Leiden University Press it will publish the Boerhaave Series for Postgraduate Medical Education. The first volume of this new series is the product of the course on Human Blood Coagulation given in Novem ber 1968. Diabeticretinopathy, corneal neovascularization, age-related macular degeneration, retinal veinocclusion, andretinopathy of prematurity are major causes of blindness. Development of lasertechnologiesandtheir wide use in ophthalmology have broughtsomehopeforthetreqtment of these diseases. However, itbecameevident that direct closureofthenewvesselsand/orablationoflargepartsof thetissuearenotidealsolutionstotheproblem. Recent advances in molecular biology and genetic engineering have fosteredbasic knowledge regarding the growth factors and intercellular messengers and their association with theproliferationofnew blood vessels. These latter findings sparked a renewed interest in Michaelson's "X-factor" ofocular neovascularization and opened avenues for possible innovative therapeutic approaches. Therefore, the suggestion to organize an international gathering of clinicians and scientists interested in problems of ocular circulation and neovascularization was received with great enthusiasm. The "shoulder-hand syndrome" or pain in the shoulder or arm is an extremely vital subject for the multi-disciplinary approach and usually re quires more than one speciality for complete evaluation. The proceedings of this symposium - organized by the department of general surgery of the University of Limburg - cover the field thoroughly with contributions from outstanding specialists from all over the world. Rheumatological, neuro surgical, orthopedic, and traumatological aspects are covered. Vascular surgeons considering both venous and arterial problems entwine with thoracic surgeons to review the thoracic outlet problem. Establishing a diagnosis in this multi-disciplinary field is extremely important and encom passes a combination of disciplines including neurology, orthopedics, sur gery, and physical medicine. Likewise, the role of each contributing factor must be assessed when multiple sources were responsible for pain. In addition to adequate diagnosis, it is important to institute the appropriate therapy and to establish the proper priority and timing of such therapies. The follow-up techniques particularly regarding thoracic outlet syndrome are extremely important to provide an objective basis for evaluation. Although many advances have been made in the field of objective diagnosis and therapy, there still is a great deal of overlap and confusion which has to be clarified. Professor Greep and his organizing committee have done a wonderful job in composing a program covering all aspects of this field. This monograph had its genesis in a workshop on the specific conduction held in the spring of 1975. The meeting was organized to discuss present knowledge on structure and function of the cardiac specialized tissues with emphasis on their clinical implications. Since much new information was presented, the participants agreed to prepare manuscripts and make their material available for publication. This has resulted in a book in which the cardiac specialized tissues are discussed by different specialists: the electron-microscopist, anatomist, pathologist, physiologist, physicist and clinician. Apart from their interest in the cardiac conduction system the participants shared the opinion that their contribu tion should be relevant to the understanding and treatment of patients with cardiac arrhythmias. The book should be useful for the clinician, the morphologist and the physio logist. The workshop took place at the University Department of Cardiology, Wilhelmina Gasthuis, Amsterdam, The Netherlands. This is the home ground of one of the most outstanding electro cardiologists of our time, Dr. Dirk Durrer. By pairing genius and originality with endless fund of energy and dogged persistence he made several important contributions to modern cardiac electrophysiology. In recent years he created a cardio logical institute where workers from various disciplines cooperate in the study and treat ment of cardiac disease. This book comprises the essential records from a Boerhaave course given in June 1971 for specialists in cardiology and other fields of internal medicine interested in the subject of quantitation in cardiology. It is evident that in the wide field of medicine, and particularly in cardiology, there is a growing need for exact and detailed information in conjunction with existing diag nostic methods. This is apparent in the greater precision in anatomical and haemodynamic details required by the thoracic surgeon as the number of available heart operations gradually increases. In retrospect it is hardly surprising that the high initial mortality after the introduction of each operation depended to a large extent upon the degree of accuracy with which the diagnosis was made. Another urgent need for precise and quantitative information became apparent when monitoring of high-risk patients, in order to forestall compli cations and/or death, became routine. The same applies for the diagnostic procedures used to estimate the patient's chances of surviving an operation and/or rehabilitation after a serious incident, such as myocardial infarction. In these fields there is a vast amount of data to be handled and - in modern diagnostic procedures - it must be processed so rapidly that the human mind cannot suffice and computer equipment has becomeindispensable. The state of health care is reflected by perinatal and neonatal morbidity and mortality as well as by the frequencies of long-term neurological and developmental disorders. Many factors, some without immediately recognizable significance to childbearing and many still unknown, undoubtedly contribute beneficially or adversely to the outcome of pregnancy. Knowledge concerning the impact of such factors on the fetus and surviving infant is critical. Confounding analyses of pregnancy outcome, especially these past 2 or 3 decades, are the effects of newly undertaken invasive or inactive therapeutic approaches coupled with the advent of high technology. Many innovations have been introduced without serious efforts to evaluate their impact prospectively and objectively. The consequences of therapeutic misadventures character ized the past; it seems they have been replaced to a degree by some of the complications of applied technology. Examples abound: after overuse of oxygen was recognized to cause retrolental fibroplasia, its restriction led to an increase in both neonatal death rates and neurologic damage in surviving infants. Administration of vitamin K to prevent neonatal hemorrhagic disease, particularly when given in what we now know as excessive dosage, occasionally resulted in kernicterus. Prophy lactic sulfonamide use had a similar end result. More recent is the observation of bronchopulmonary dysplasia as a complication of re spirator therapy for hyaline membrane disease. It was the genius of Gordon Murray in Toronto that introduced the use of allografts into cardiac surgery in the 1950s. Soon after this on opposite sides ofthe world, Sir Brian Barratt-Boyes in Auckland, New Zealand, and Mr. Donald Ross in London, undertook to use allografts for the replacement of diseased aortic valves. Since that time the global interest in allografts has been patchy, episodic, and without a con sensus. Nonetheless, for the last 20 years at least three groups in the world have steadfastly pursued the development of new and relevant information concerning the use of allograft valves in humans. These are the centres of Sir Brian Barratt Boyes, Mr. Donald Ross, and Mark O'Brien in Brisbane. More recently, talented investigators, including Drs. Yankah, Yacoub, and others, have been developing information concerning the immunological aspects of the use of allografts, as well as their clinical use. No doubt, at present, cardiac valve allografts of one sort or another are the devices of choice for conduits and have an important place in the surgery of aortic valve replacement. Even so, in the mind of this writer at least, the future usefulness of allografts for the replacement of diseased cardiac valves and conduits between a ventricle and the pulmonary artery, remains problematic, and depends upon improvements in other devices for this purpose and upon improve ments that may be made in preparing and using allografts. Vom pathophysiolgischen Verständnis ausgehend erlebten andere Indikationsgebiete, insbesondere die thrombolytische Therapie des Myokardinfarktes und arterieller Verschlußkrankheiten in den letzten Jahren eine Renaissance. Die Vielzahl der Kontraindikationen und die Angst des Therapeuten vor Blutungskomplikationen regten die Suche nach Medikamenten mit hoher Wirksamkeit am Ort des thrombotischen Verschlusses bei gleichzeitig geringem Einfluß auf das Gerinnungssystem an. Nachdem mit der Entdeckung humaner Enzyme mit thrombolytischer Wirksamkeit und hoher Affinität zu Fibrin die wissenschaftliche Basis erarbeitet war, gelang es zunächst, diese Proteine aus Organextrakten und später aus Zellkulturen in kleinem Maßstab herzustellen. Den entscheidenden Fortschritt brachten gentechnologische Verfahren, die die großtechnische Produktion und die Herstellung ausreichender Mengen reiner biologisch wirksamer Proteine erlaubten. Das Buch enthält die Beiträge zahlreicher Wissenschaftler auf dem Gebiet der Fibrinolyse, die in einem Erfahrungsaustausch über den Einsatz von Medikamenten berichten und die Ergebnisse der bisher durchgeführten experimentellen und klinischen Studien kritisch diskutieren und bewerten.

Also discussed is the assessment of the effectiveness of standard treat ment and how it effects the quality of a patient's survival. In the last two decades, there has been a major increase in the actual numbers of women diagnosed with breast cancer (from 65000 new cases 20 years ago in the United States to 183000 in 1993). Concomitant with this has been a shift toward making the diagnosis earlier and toward diagnosing an earlier stage ofthe disease (more than 70% of diagnoses were of advanced-stage disease two decades ago, whereas more than 70% of diagnoses are now earlier-stage disease in 1992). These changes are clearly related to the woman's greater sensitivity to abnormalities in the breast and her seeking medical care, to the physician's greater awareness of the potential diagnosis of cancer, and to the major and important impact of mammographic screening. These changes have had a major impact on the questions of appropriate treatment, thereby contributing to the controversies in the disease. What is the role of surgery? What are the respective advantages of radical mastectomy, modified radical mastectomy, and conservation surgery with and without supplementary treatment? What is the role of radiation therapy? When should radiation therapy be performed after mastectomy, and what volumes should be irradiated post mastectomy? Where is radiation therapy indicated after conservation surgery, and when is radiation therapy an effective palliative treatment measure? Radiotherapy of Intraocular and Orbital Tumors presents a new approach to the role of radiation therapy in the management of ophthalmic cancer. Particular attention has been paid to clinical presentation under acute, subacute or chronic forms, to the appearance of initially aspecific symptoms evolving into a more characteristic syn­ drome at a later age, or vice versa, to clinical heterogeneity and its relation to genetic and biochemical heterogeneity. Brief reference to other diagnostic possibilities is also given in all disease-related chapters. Description of the metabolic derangements is restricted to the main pathophysiological features which provide the rationale for diagnosis and treatment. Methods to ascertain the diagnosis and diagnostic tests are listed comprehensively. As in the first edition, treatment is discussed extensively. Details are given for dietary treatment and drug administrations in acute situations, during infections and in maintenance treatment. The impressive progress of knowledge with respect to genetic lesions in inborn errors of metabolism remains condensed to the essentials. For more detailed information, particularly with respect to pathophysiology and genetics, we highly recommend the seventh edition of The Metabolic Basis of Inherited Disease, by Charles R. Scriver et al. (McGraw-Hill, 1995).

The surgical resection of a major portion of the liver for cancer of this viscus has long been neglected. Although the first successful resection of a primary malignant tumor of the liver was performed by LUCKE in 1891, this pioneer effort failed to stimulate interest in the operative removal of hepatic neoplasms. Earlier surgeons were intimidated by the concept of the liver as a huge vascular sponge possessed of three intricate circulatory systems. To the fear of uncontrollable hemorrhage was added the supposed hazard of hepatic insufficiency following the removal of a large portion of an organ with so many vital functions. Last but not least was the difficulty in surgical exposure. Only in recent years has the sporadic interest of surgeons been changed to a growing concern about the possibilities of extirpation of hepatic tumors by the per formance of major hepatic lobectomies. Four factors have contributed to this: 1) the dissemination of more accurate information about the surgical anatomy of the intrin sic and extrinsic hepatic circulatory systems, which has acquainted surgeons with the principles and techniques of hemostasis essential for successful operative intervention on the liver; 2) increased knowledge of the functions and regenerative capacity of the liver; 3) greater diagnostic skill aided by modern laboratory procedures; 4) improved surgical exposure of the liver by the use of laparothoracotomy incisions. Such mortality is but the tip of an iceberg of morbidity that saps the quality of life of a majority of this world's citizens. They are the less privileged by accident of birth only. An intolerable situation exists. These Proceedings reflect a world concern by m~n~s ters of health, academicians, scientists and providers of maternal and neonatal care. No blueprint is offered for solution of this world's greatest health problem. But the Proceedings do contain an accurate description of the problem and revealing epidemiological diagnoses that both lay bare deficiencies in care and suggest the way to im prove "health for all". The implications of these papers will not bring comfort to those responsible for the allocation of scarce resources for health. They rather suggest radical changes are needed and that failure to make these changes could mean a catastrophic and meaning less loss of life during the balance of this century. The concept of the foeto-placental unit as an integrated endocrine organ has been defined recently by many in vivo studies at the 17th- 20th week of gestation. A functioning foeto-placental unit is necessary for most of the increased oestrogen production of pregnancy and for the provision of glucocorticoids and aldosterone to the foetus. Neither the foetus nor the placenta alone have the necessary enzyme systems for the synthesis of these groups of steroids. However, when the foetus and placenta function as a unit, all of the enzyme systems are present for the synthesis of these steroids from circulating cholesterol. The placenta, but not the mid-gestation foetal adrenal, can synthesize physiologically significant amounts of pregnenolone from circulating cholesterol. Part of the pregnenolone is converted to progesterone in the placenta by the 3~-HSD system (absent in the foetus). The progesterone is transferred to the foetus where it is transformed by C-II, C-17, C-18 and C-21 hydroxylases (all absent in the placenta) to cortisol, corticosterone and aldosterone. Pregnenolone transferred from the placenta to the foetus undergoes 171X-hydroxylation, side chain splitting and sulfurylation (absent in the placenta) and is converted to DHAS. The DHAS may undergo 161X-hydroxylation (absent in the placenta) in the foetal liver and be transported to the placenta as 161X-OH-DHAS. There it is subjected to a neutral steroid sulfatase (absent in the foetus) and is converted to oestriol by action of the 3~-HSD system and the aromatizing enzyme system. The course of history is never one of smooth progression. Periods of relative quietness are interrupted by periods of wars and revolution. This pattern resembles that of a river which, before flowing into the delta, has to pass countless rapids. The same holds for the development of the science of medicine. In obstetrics some of these 'revolutions' or 'rapids' consist of the introduction of conservative obstetrical treatment by Lucas Johann Boer at the beginning of the nineteenth century, the discovery of the cause of puerperal sepsis by Oliver Wendell Holmes and Semmelweiss between 1843 and 1847, the introduction of the principle of asepsis by Pasteur in 1874, the introduction of prenatal care at the end of the nine teenth and the beginning of the twentieth century (Mijnlieff, Treub, De Snoo), the improvement of surgical techniques, the possibility to treat shock by bloodtransfusion, and, finally, the acquisition of new means for the effective therapy of infection. All these developments have led to a sharp reduction of maternal and perinatal mortality. In this connection it must be pointed out that such a reduction could never have been accomplished without the favourable social changes as a result of which medical and prenatal care could be made universally available. In recent years there has been another revolutionary develop ment in obstetrics. Two factors have been responsible for this: the ap plication of basic sciences in obstetrics, and the dissolution of the isolation with respect to other clinical disciplines. The most common cause of death in the civilized countries remains coronary heart dis­ ease, a multifactorial illness which might be best treatable by prophylactic means. In spite of improvements in diagnosis, drug therapy, monitoring, and particularly surgery morbi­ dity and mortality of our patients oblige us to look for new frontiers in understanding the pathophysiology of the disease and for new approaches in the treatment of myocardial dysfunction, the limiting factor in the activity of patients surviving an acute myocardial infarction. Salvage of viable myocardium still seems to be the most important challenge in the treatment of acute ischemia. Lysis therapy to reopen occluded coronary arteries seems to be promising in a great number of patients. However, several experimental re­ ports of reperfusion injury and clinical series with a considerable number of patients, who do not benefit from this intervention, need alternatives to reassess myocardial structure and function. The necessity to find solutions to this problem coincides with the develop­ ment of an alternate access route to deprived coronary vasculature. Starting with the ideas and results gathered in the last 60 years, Meerbaum and Corday in Los Angeles and our group here in Vienna developed two different approaches to myocardial protection via the coronary sinus.

This book contains the papers delivered at the Fourth International Symposium on Intracranial Pressure, held at Williamsburg, Virginia, USA, June 10-14, 1979. Divided into 12 sessions, they reflect the most recent developments in areas such as head injuries, pressure volume studies, cerebrovascular complications, intracranial hemor rhage, brain edema, systemic factors and infectious processes, data recording and analysis, CSF formation and absorption, hydroce phalus, clinical aspects of ICP monitoring, anesthesia and intra cranial pressure, treatment with barbiturates and steroids, and os motherapy. The book concludes with a summary of the present state-of-the-art in the field as a whole by Dr. Langfitt. There were two innovations at this Symposium. The first of these was poster sessions, the second, breakfast seminars. This volume contains all papers read plus all those presented as posters, and for this reason contains more pages than the three previous volumes. The organizers wish to thank the Advisory Committee for the work done in paper selection and focus of the Conference. Appreciation is also given to the Chairmen and Co-chairmen of the sessions for the preparation of summary statements.
